As a courtesy, we will contact your insurance company and verify coverage and benefits. Telephone verification of insurance benefits is not a guarantee of payment. Your patient responsibility may change if the insurance payment is different or if the billed amount changes.

We will file with your insurance company for the facility charges. Please remember that you are responsible for any outstanding balance not paid by your insurance company. All account balances are to be paid within 90 days from the date service is rendered. IT IS NOT OUR INTENT TO OFFER OR EXTEND CREDIT FOR MEDICAL SERVICES RENDERED. Letters of protection are not acknowledged.

For your convenience, we accept VISA, MASTERCARD, DISCOVER, AMERICAN EXPRESS as well as a personal check, money order or cash. A $25.00 fee will be charged on any returned checks.

Please be advised that the facility fee does not include the following professional services:

• Professional Services of the Operative Surgeon
• Professional Services of the Anesthesiologist (if applicable)
• Professional Services of the Pathologist or Radiologist (if applicable
• Laboratory Services (if applicable)
